Some SAML error is because of the way for example ServiceNow is provisioning accounts and they have to be a web integration instead of SAML because of the different groups it’s being setup with. Service Now Prod Provisioning is setup for everyone to have an account but the chiclet is disabled. As to the regular ServiceNow Prod is giving access to the accounts
Reports show what accounts have been at risk for example if Okta has been accessed from another location another device or country and there is such things as velocity checks as in the distance of when those devices has pinged from another location. Could it be VPN? because it is impossible for the user to ping from California then New York within 2-3 minutes unless a VPN was involved that is where the velocity check comes in to play
