talk by Laura Niklason, Yale.

cells + elastin + collagen

collagen: confers the vast mech strength that arteries need.

started in mid 1990s (she was a post doc at MIT)

PGA... use as scaffold: seed with SMCs.

1999 in Science: bovine and porcine.

design requirements:

mechanics: must be stupendous... 3 billion times (pulses)

low thrombogenecity.

an autologous cells (otherwise immunogenic)

SMCs from old humans do not grow as well as young animals.

focused on mechanics: collagen is really important;

17% of dry weight is type 1 is collagen.

grow in young animal and wash the cells away.

first app: 6 mm: high blood flow; less clotting;

2004 spin out Humacyte ealry 2005;

42 cm in length and 6 mm in diam. grow in bioreactor.

decellularized....

burst strength > 3000 mm (like rupture str of native artery!

first app:

dialysis access: 419K patients....

most PTFE grafts fail at 12 months.

we tested initially in dogs and pigs but went to baboons (to avoid immunicity)

put into 14 baboons; and poke with needles.

83% patency rate

baboons pull out stitches....

no cells pre-implant....

at 6 months gets repopulated with SMCs.

hostperceives as native matrix. get regenerated host tissue.

we did three phase 1s in Poland total of 80 pateints

implanted in 2012.

poked 3X per week for dialysis.

used for two years! in dialyssi pts.

AHA Nov 2013...

100% patency! at 6 months.

no immune resoonse....

one liter per min flow rates.... v encouraging...

(she's worked on this for 20 years)... working for 2 years.

other conduits: trachea; they last for 2 months

(our lungs only last for 2 hours)

.
