To get them into PB.

The way that I solved this was by first importing them into Outlook and from there to PB.

How to setup a new Contact folder within Outlook: see here

Prepare the .csv file:

Ctl-H (replace), replace all " (quote marks) with nothing. This will eliminate them. Then Data> Text to Columns, Delimited, Select Comma. This will separate everything into columns. Save (as .csv) and close.

Within Outlook, File > Import > Import from another program file > Comma Separated Value > File to Import > Browse for .csv > Select destination folder (created using the instructions found in the link above, in my case Linked-in Contacts) > Place a checkmark next to Import "filename.csv" into folder... > Map Custom Fields (This is to confirm that the values are in the correct fields within Outlook). (Click on the Next button to see how the values look when mapped). Finish.

The contacts should now be in Outlook. Next, select all contacts and drag&drop into the Parent thought that you would like in PB.
