Naritriptan (Amerge)
The longest acting (as is Frovatriptan)
Naritriptan, commonly known by its brand name Amerge, is a medication primarily used in the treatment of migraine headaches. It belongs to a class of drugs known as triptans, which are specifically designed to treat migraines by targeting serotonin receptors in the brain.
MECHANISM OF ACTION
Naritriptan works by stimulating serotonin (5-HT) receptors in the brain. Specifically, it targets the 5-HT1B and 5-HT1D receptors. By activating these receptors, Naritriptan causes the blood vessels in the brain to constrict (vasoconstriction), which helps alleviate the headache and other symptoms associated with migraines. Additionally, it reduces the release of pro-inflammatory neuropeptides, further contributing to its effectiveness in treating migraines.
Uses
Naritriptan is primarily prescribed for the acute treatment of migraine attacks with or without aura in adults. It is not intended for the prevention of migraines or for the treatment of cluster headaches. The medication is most effective when taken at the first sign of a migraine attack, allowing it to work quickly to relieve symptoms such as headache, nausea, and sensitivity to light and sound.
Dosage and Administration
Naritriptan is available in tablet form and is typically taken orally. The standard initial dose is 1 mg or 2.5 mg, depending on the severity of the migraine and the patient's response to the medication. If the migraine persists or returns, a second dose may be taken at least four hours after the first dose. However, the total daily dose should not exceed 5 mg. It is important for patients to follow their healthcare provider's instructions regarding dosage and administration.
Side Effects
Like all medications, Naritriptan can cause side effects. Common side effects include:
- Dizziness
- Drowsiness
- Fatigue
- Nausea
- Dry mouth
These side effects are generally mild and tend to resolve on their own. However, some patients may experience more serious side effects, such as:
- Chest pain or tightness
- Shortness of breath
- Irregular heartbeat
- Severe stomach pain
- Allergic reactions (rash, itching, swelling)
If any of these serious side effects occur, it is crucial to seek medical attention immediately.
Precautions and Contraindications
Before taking Naritriptan, patients should inform their healthcare provider of any medical conditions or allergies they have, as well as any other medications they are taking. Naritriptan is contraindicated in patients with:
- A history of heart disease or stroke
- Uncontrolled high blood pressure
- Severe liver impairment
- Peripheral vascular disease
Additionally, Naritriptan should not be used within 24 hours of taking another triptan or ergotamine-containing medication, as this can increase the risk of serious cardiovascular side effects.
Drug Interactions
Naritriptan can interact with other medications, potentially leading to adverse effects. Some notable interactions include:
- MAO inhibitors: These can increase the levels of Naritriptan in the blood, leading to an increased risk of side effects.
- SSRIs and SNRIs: These antidepressants can increase the risk of serotonin syndrome, a potentially life-threatening condition.
- Other triptans or ergotamine derivatives: Using these medications in conjunction with Naritriptan can increase the risk of cardiovascular side effects.
Patients should always inform their healthcare provider of all medications and supplements they are taking to avoid potential interactions.
