Hepatitis B
Chronic HBV infection is defined as the persistence of hepatitis B surface antigen and serum HBV–DNA levels detectable for at least 6 months. Globally, CH-B affects approximately 400 million people worldwide and contributes to more than one million deaths annually of liver disease (Lavanchy, 2004). CH-B has become one of the major causes of chronic hepatitis, cirrhosis, and hepatocellular carcinoma worldwide. For patients with active viral replication, cirrhosis will be observed in 15–20% of patients within 5 years. Among patients with cirrhosis, the disease may progress, and the incidence of hepatocellular carcinoma is significantly elevated. 70–90% of patients with hepatocellular carcinoma have cirrhosis (Karayiannis and Thomas, 2005). It has been found that hepatitis B has a markedly negative impact on health-related quality of life, both in terms of physical function and mental health.
The World Health Organization has estimated that some 257 million people are infected with the HBV globally, with the virus having been associated with nearly one million deaths in 2015 alone, mainly due to complications such as cirrhosis and liver cancer. In the absence of curative treatment, the most common treatments for the HBV include IFN-α and nucleotide analogues, which often lead to severe side effects and drug resistance issues, while having little effect in terms of treating liver fibrosis, cirrhosis, or liver cancer.
In China, over 90% of chronic hepatitis B (CHB) patients received the traditional Chinese medicine (TCM) therapy. Studies have shown various types of TCM to target the HBV, liver fibrosis , or hepatocellular carcinoma. Further, a meta-analysis of clinical trials found that TCM had an equivalent or better effect than interferon or lamivudine in the treatment of CHB . Moreover, randomized controlled clinical studies have reported the effects of different TCM formulae with regard to improving the clinical parameters of patients with liver fibrosis.
A large long-term population study identifiesS. barbata among the single herbs used in TCM that may reduce risk of hepatocellular carcinoma in hepatitis B patients.
